Graduating from the Transitional Housing Program is a significant accomplishment, but it is only the beginning of a long journey for Sojourner House women.
During their stay at Sojourner House, they worked hard. They continued in their recovery programs for chemical dependency and/or mental illness, moved toward family unity, and gained critical life skills to aid them in the future. Homeless when they arrived, they leave Sojourner House with a personal goal plan and move into one of Sojourner House’s 40 supportive housing apartments or safe, affordable housing in the community.
Sojourner House maintains strong ties with graduates who choose to move directly into the community and provides long-term support through its Outreach Program. Serving as a surrogate family, Sojourner House walks side by side with each woman on her journey to self-sufficiency.

Sojourner House Family Outreach Coordinators provide on-going-consistent support for to the women and families served in its Outreach Program. Support services provided families include:
- Individual case management
- Referrals to area agencies and organizations as needed
- Family unification support

Sojourner House’s long-term support provides hope for the future and helps graduates realize their dreams. Many of them go on to complete their education, advance in their careers, and become homeowners.
