Sojourner House offers a continuum of housing and support programs for women and their children, including:
  • A six-month transitional housing program for 16 women and their children.
• Permanent, affordable housing with on-site case management for 40 families.
• Follow-up case management and support services to hundreds of former residents living in the community.
Women begin their journey in our transitional housing program that provides:
  • A safe, structured environment in which to begin their journey toward lasting stability
• Intensive case management and counseling support provided by our staff
• Comprehensive life skills traning through the Henrietta Hammond Institute for Life Skills (HHILS) Program

Upon successfully graduating from the six-month program, women either move into one of our supportive living apartments (where they may stay for up to four years) or receive assistance securing safe, affordable permanent housing in the community. And, for as long as needed, ongoing case management and support services are provided through our supportive housing and outreach programs, providing the following support to over 200 women and their families:

 

• Case management (i.e. weekly one-on-one counseling, goal setting, crisis intervention)
• Referrals to area agencies and organizations for individual needs (i.e. medical, mental health, drug and alcohol rehabilitation, financial counseling)
• Family unification support (i.e. Child Protective Services liaison, child care needs, parenting education referrals)
• Monthly resident community meetings (for those in our supportive housing)
• Enrichment programs for children through our Dreamseeds program (i.e. teen group therapy, arts and cultural opportunities, one-on-one tutoring twice per week)
• Special events and holiday celebrations at Sojourner House (i.e. Graduation Picnic to celebrate accomplishments, Back to School picnic where school supplies are given out to children, Brunch With Santa where Christmas gifts are distributed)

We believe the long-term success of Sojourner House families in maintaining self-sufficiency and family stability relies upon the long-term support provided by our supportive housing and outreach programs, where our graduates live independently, but receive strong and sustained support from Sojourner House. This continuum of care approach recognizes the ongoing, daily struggles of Sojourner House women to remain in permanent housing, maintain their sobriety, achieve stable employment, and keep their families together.
