Some graduates of Sojourner House opt to find housing in another Supportive Housing program or in an apartment on their own in the community at large. Through our Outreach Program, the support clients receive from Sojourner House doesn’t end. As long as they need it, Sojourner House will be there for them, a support system to always count on.
During their initial recovery period, these clients of Sojourner House and other programs worked hard. They participated in their programs for chemical dependency and/or mental illness, moved toward family unity, and gained critical life skills to aid them in the future. Homeless when they started, they now have a personal goal plan and the confidence that they can succeed. Case managers maintain strong ties with graduates and women from other community programs who choose to move directly into the community. Serving as a surrogate family, staff walk side by side with each woman on her journey to self-sufficiency. Sojourner House’s long-term Outreach support provides hope for the future and helps graduates realize their dreams. Many go on to complete their education and advance in their chosen careers.

Sojourner House Family Outreach Coordinators provide on-going-consistent support for to the women and families served in its Outreach Program. Support services include:
- Individual case management
- Referrals to area agencies and organizations as needed
- Family unification support
- Special events and holiday celebrations at Sojourner House
- Educational and enrichment programs for children.

Shelter Plus Care Outreach
Funded by a joint HUD grant with the YWCA and administered by the Rochester Housing Authority, Sojourner House at PathStone provides case management and supportive services to residents at various locations throughout the city of Rochester, New York. This program enables individuals and families to maintain subsidized supportive housing in the community at large.
